The goal isn't to convince you that herbal medicine is right for you. What is herbal medicine and how do herbs actually work?

You already know plants can affect the body. Coffee can make you feel more alert. Fiber can change digestion. Herbal medicine begins with that same basic idea: plants contain compounds that can influence how the body functions. But herbal medicine is not just about taking a plant and hoping it helps. It is about understanding which plants, in what form, in what combination, and for which person.

For thousands of years, people across nearly every culture have turned to plants for healing [1]. Long before modern pharmaceuticals existed, entire medical systems were built around herbs. And even today, many conventional medications trace their origins back to plant compounds—aspirin from willow bark, for example, or the cancer drug paclitaxel from the Pacific yew tree [2].

If plants can affect the body, the next question is obvious: how?

Plants may look simple from the outside, but chemically, they are anything but passive. They produce a wide range of chemical compounds to survive—defending against insects, adapting to environmental stress, and regulating their own growth. When we consume certain plants, some of these same compounds can interact with our bodies in meaningful ways. That’s what people mean when they say herbs are “biologically active.” They don’t work simply because they’re natural—they work because they contain compounds that can influence biological processes.

This complexity is part of what makes herbal medicine both fascinating and challenging. Some herbs have been studied extensively, while others are still being actively researched. Some are very safe when used appropriately, while others can interact with medications or may not be suitable during pregnancy or for certain health conditions [3].

If herbs work, why isn't there more research?

At this point, you might ask: if plants can affect the body, and they have been used successfully for thousands of years, why aren’t there large clinical trials proving exactly which herbs work for every condition? The answer isn't as straightforward as you might expect.

There is research, but the picture is nuanced. Some herbs have been studied extensively and have strong evidence for certain uses. Others have only been evaluated in small clinical studies or laboratory research [3]. For many herbs and herbal formulas, researchers are still building the evidence base. 

One reason is that herbal medicine is often practiced differently than conventional medicine. In a clinical trial for a medication, every participant with the same condition typically receives the same treatment. Practitioner-guided herbal medicine is often more individualized. Two people with the same diagnosis may receive different formulas because their symptoms, health history, medications, and overall presentation are different. While this individualized approach is one of herbal medicine's defining characteristics and strengths, it also makes it more difficult to study using research methods that often work best when one standardized treatment is given to every participant [4].

Herbal formulas add another layer of complexity. Conventional medications often contain a single purified active ingredient. By comparison, a single herb may contain dozens—or even hundreds—of naturally occurring compounds, and many formulas intentionally combine multiple herbs that each serve a different purpose. Studying these complex formulas is possible, but it is often more challenging than studying a single purified compound.

Another factor is consistency. Pharmaceutical drugs are manufactured to contain the same active ingredient in the same amount every time. Herbal products can vary depending on the plant species, where and how the plant was grown, when it was harvested, and how it was prepared or extracted [5]. Modern quality standards have improved consistency considerably, particularly among reputable manufacturers, but these differences can still make research more complicated.

Finally, large randomized clinical trials are expensive regardless of what is being studied. Developing a new pharmaceutical may justify that investment because companies can often obtain patents or other forms of market exclusivity. Whole plants generally cannot be patented in the same way as newly developed drugs. That means there is often less commercial incentive for private companies to fund the large-scale research needed to answer every question about herbal medicine.

None of this means herbal medicine should be accepted without evidence. It means the evidence base often looks different than it does for prescription medications. For you, the most useful question is not whether “herbs” work in general, but what is known about a specific herb or formula, for a specific concern, given your health history and medications. That is why responsible herbal care considers published research, traditional knowledge, clinical experience, product quality, medications, and the individual’s overall health picture.

This is not the same as supplements

This is where it’s easy to get the wrong impression: you try a supplement and assume that experience represents the whole field.

A supplement is usually a finished product. It already exists before you buy it. The ingredients, dose, and directions are set in advance, and the same product is sold to many different people with very different bodies, medications, symptoms, and goals [6].

Practitioner-guided herbal medicine works differently. The formula doesn’t come first—the assessment does. Before anything is prepared, the practitioner reviews your symptoms, health history, medications, sensitivities, and treatment goals. That assessment determines what herbs to use, how they should be combined, what preparation makes sense, and how the formula should be dosed.

Rather than selecting a pre-made product, practitioners often create formulas on demand using multiple herbs in precise ratios. Because the formula is tailored, it can also change. As your symptoms shift or new information emerges, the practitioner can adjust the composition, concentration, or dosing strategy without being limited to what exists on a shelf.

That’s why comparing practitioner-guided herbal medicine to supplements is misleading. Supplements are products. Herbal medicine, in this context, is a clinical process: assessment, formulation, preparation, follow-up, and adjustment.

Why people choose herbal medicine

By the time you consider herbal medicine, you're usually not looking for another product. You're looking for a different approach to your health.

For many people, one reason is simple: they would prefer to support their health with plant-based medicine whenever it is appropriate to do so. They're drawn to the idea that herbs are derived from whole plants, minimally processed, and have been used across cultures for thousands of years. Others arrive after they've already done the reasonable things: seen a doctor, ruled out urgent concerns, tried diet changes, taken medications, or experimented with supplements. Sometimes those steps help. Sometimes they don't explain why your symptoms keep returning or why you still don't feel like yourself.

Choosing plants is often what brings people to herbal medicine. The way practitioners think is what keeps them there.

Imagine you and someone else both complain of bloating. You sleep six hours a night, feel constantly stressed, and develop symptoms after most meals. The other person sleeps well but only reacts to dairy. You share a symptom, but not necessarily the same underlying pattern. Herbal medicine starts there.

Instead of asking only, “How do we reduce this symptom?” a practitioner asks, “What is contributing to this, and what systems need support?” Digestion, sleep, stress, energy, medications, lifestyle, and health history may all be part of the picture.

This is what people mean when they describe herbal medicine as holistic: your practitioner looks at you as a whole person, not just one complaint. The goal is not simply to suppress a symptom, but to support your body's broader systems.

This doesn't mean herbal medicine is better than conventional medicine, or that conventional medicine only suppresses symptoms. Many medications are essential, effective, and lifesaving. Herbal medicine simply offers a different philosophy of care—one that uses plants thoughtfully, looks at the whole person, and is designed to evolve over time.
